Expanding Access Through Community Partnerships

One of the most exciting aspects of Vision Forward’s future is not simply expanding services—it is bringing those services closer to where people already receive their eye care.
This month, we are proud to highlight our growing partnership with Eye Physician Associates, another important step in expanding access to specialized low vision rehabilitation throughout southeastern Wisconsin.
By embedding Vision Forward clinicians within ophthalmology practices, we reduce barriers to care, strengthen collaboration between medical providers and rehabilitation specialists, and make it easier for individuals to receive comprehensive support following a diagnosis of vision loss.
This partnership showcases Vision Forward’s successful model of satellite and pop-up clinics, allowing more individuals to receive life-changing services closer to home.
As Dr. Hirsch recently shared: “Eye Physician Associates is proud to bring the much-needed low vision services provided by Vision Forward to our Greenfield office. We hope this location allows patients who need low vision care more convenient access to the services they need.”
Together, we are creating a more connected continuum of care that bridges medical treatment with rehabilitation, helping patients maximize their remaining vision and maintain independence. Thank you, Eye Physicians Associates, for your outstanding partnership! Your commitment to your patients extends far beyond the exam room, and we are deeply grateful to work alongside you in transforming lives and expanding access to the care and support every individual deserves.
